सेवा में comes first with the recipient's designation, then the date, then विषय saying what the letter is about, then महोदय, then the body, then भवदीय with the name. The विषय line is the part children most often leave out entirely - and it carries a mark of its own in a formal letter.

प्रधानाचार्य को → औपचारिक, greeting महोदय. The test is the relationship, not the importance: anyone you know personally takes an informal letter, anyone in an official role takes a formal one. An elder brother is आदरणीय but still informal, which is the item children most often get wrong.

The model application has every part in order, and the body is only two short paragraphs: why leave is needed, and the request itself. Children often write five paragraphs of explanation - the marks are for the format and for a clear reason, not for length. सविनय निवेदन है कि and अतः आपसे विनम्र निवेदन है are the two phrases worth learning by heart.

The relationship decides it. Someone you know personally takes an informal letter with प्रिय or आदरणीय; someone in an official role takes a formal one beginning सेवा में with a विषय line and महोदय.
